Important COVID-19 Updates for Our Patients!

At Suburban Research Associates (SRA), safety is our top priority. As efforts concerning the containment
of COVID-19 (Coronavirus) expand and evolve here in the U.S and around the world, we want to assure
you we are focused on three things:

• Ensuring the safety of our patients, employees, and our community
• Continuing to provide exceptional service to our patients.
• Supporting local efforts to limit the spread and impact of COVID-19.

As many of you know, many companies have relaxed their facemask restrictions in the region and we
have been looking closely at the data being published by the Department of Health regarding the
number of positive COVID-19 cases as well as the vaccination rate. Although there had been a
significant decrease in the number of positive COVID-19 cases, the most recent data published from the
Department of Health saw the first increase in cases in several weeks.

As a result of the recent increase in positive COVID-19 cases we are not ready to lift the mandatory face
mask requirement within our organization.

Facemasks must be worn by all employees and patients in the following circumstances and regardless of the person’s vaccination status:

1. Employees must wear face masks that completely cover the nose and mouth when in any
common area of a building within the organization.
2. Employees who are providing in-person services must wear face masks when delivering services
to or engaging with participants.
3. Patients receiving services must also wear facemasks that completely cover the nose and mouth
when inside of Suburban Research Associates and Sequioa behavioral Healthcare regardless of
the person’s vaccination status.

We will continue to review data published by the Departments of Health for information regarding
changes in the number of positive COVID-19 cases along with the number of persons vaccinated. Please

COVID-19 Mitigation Policies

General Facility

1. COVID-19 sanitation protocol and social distancing plans are in effect throughout the office.
2. The office is cleaned and disinfected weekly. Exam rooms are cleaned and sanitized after EVERY
use. Air filtration systems with UV light have been placed on both floors of the office for added
protection and sanitation.

Staff Mitigation Process

1. All staff members on-site are required to wear a face mask or covering when completing study
participant visits.
2. Staff members are encouraged to wear gloves during study participant visits and wash their
hands regularly.
3. Staff members are expected to follow the latest CDC guidance at the link.

Pre-Screening Study Participant Mitigation Process

All pre-screening visits are conducted remotely, via telephone.

Participants who qualify for a study will be scheduled for an on-site screening visit. They are advised to contact a staff member to reschedule their visit, if they have experienced any of the following:

a. Temperature of 100°F or greater anytime within the previous seven days.
b. Coughing, sore throat, shortness of breath, or any flu-like symptoms.
c. Close contact or exposure to an individual who tested positive for COVID-19 within the last 72 hours.

Study Participant Mitigation Process

1. All study participants are called or texted the day before their appointment. They are advised to contact a staff member to reschedule their visit, if they have experienced any of the following:

a. Temperature of 100°F or greater anytime within the previous seven days.
b. Coughing, sore throat, shortness of breath, or any flu-like symptoms.
c. Close contact or exposure to an individual who tested positive for COVID-19 within the last 72 hours.

2. . All study participants are asked to wear facial coverings to their on-site appointments. They are provided with a mask if they do not arrive to the office wearing a mask.
4. Remote visits are offered to study participants only when study protocol allows.

Sponsor or CRO Monitors/Staff Mitigation Process

1. Our office is accommodating both remote and on-site monitoring visits at this time.
2. All sponsor/CRO staff are asked to wear facial coverings to their on-site monitoring visits. They are provided with a mask if they do not arrive to the office wearing a mask.
